Death Certificates
- Order certified copies of the death certificate from the county where death occurred.
- Each Certified Death Certificate is $20.00 each.
- Bleitz Funeral recommends the family orders them — they will arrive more quickly.
- Please allow 10–14 business days for delivery, if funeral home orders.
Below is a list of places that may require a certified copy (no copies):
- Social Security Administration (800-772-1213)
- Transfer of real property, houses, lots, etc.
- Settling insurance claims – one for each company
- Obtaining union benefits
- Transfer of automobile, trailer, boat or camper
- Transfer of stocks or bonds – one for each corporation
- Transfer of bank checking, savings or trust accounts
- Transfer of credit union accounts
- Settling mortgage insurance claims
- Settling claims on insured loans and credit cards
- Filing federal tax returns

4 methods to order a death certificate
Print out the form, write in your information, and mail it to the address provided on the form. You can expect your death certificate order in approximately 2-3 weeks when choosing the mail-in method.
Online
Visit the VitalChek website then follow the instructions on the site. All credit card orders are processed within 2-3 days and then sent by regular mail.
UPS overnight delivery
UPS (next-day delivery) is an additional $20.00 and requires a signature when delivered. No UPS delivery to PO boxes.
A fee of $12.00 is added per order (for any quantity of records in the order) for expedited handling; e.g. if you are ordering 1 birth certificate at $20 + $12 handling fee, you can expect a total of $32.00 charged to your credit card. If you are ordering 2 copies, the charge is $40 + $12 = $52 total charged to your card.
Fax
Print out the form, fill in the blanks including your credit card information and fax the form to
206-296-0983. All credit card orders are processed within 2-3 days and then sent by regular mail. The option for UPS (next-day delivery) is an additional $20.00.
